I hereby authorize HouseholdStaffing.com (hereafter “the Company” or “Employer”), its employees, agents, private investigators or any representative of the aforesaid company, to perform investigations into my background, past behavior, to my character, general reputation, and mode of living. In addition, I further authorize investigations of the following:
Investigative Consumer Reports: I authorize the Company to perform investigative consumer reports that may include credit reports, criminal history or arrest records, workers’ compensation histories, motor vehicle records, employment and unemployment records, military records, or other sources of information.
Education: I authorize schools, colleges and all scholastic institutions to release any and all information requested. This includes transcripts, grades, attendance records, and any other information requested.
Employment: I authorize all former and current employers to release any and all information regarding my employment history. This includes all information contained in my personnel file, salary history, condemnations, and all other pertinent information. I further authorize my supervisors and other work associates to disclose their opinions and observations of my work habits, qualities, competency, and skills. Furthermore, I authorize full disclosure of any and all drug and alcohol testing results.
Authorization to Release: I authorize custodians of the records of any agency, government agency, or company as described above to release such information upon request of any investigator, agent, or representative of the Company. I understand that any or all of these investigations or inquiries can be performed prior to and periodically throughout the duration of my employment.
10-Panel Drug Screening: I give my permission to undergo a 10-Panel Drug Screening Test and I am aware that the results of all screenings will become the property of the Company and will become a permanent part of my application at the Company.
Re-disclosure: I understand that the information requested is for the use by the Company and may be re-disclosed only as authorized by law. I understand that I have the right to request from the Company a written disclosure of the nature and scope of the investigation conducted that I authorized above.
Indemnification: I indemnify, release, and hold harmless the Company, any agents of the Company, or others reporting to or for the Company, any investigators, all former employers, reporting agencies, and all those supplying references and character references, from any and all claims, defamation, demands, and/or liabilities arising out of, or related to, such investigations, disclosures, or admissions.
Signature: Copies and facsimile transmissions of this authorization that show my signature are as valid as the original release signed by me.
Many of HS.com’s clients are prominent, well-known individuals in the area. Their privacy is very important to them. A candidate hears many personal family matters discussed in the employer’s home. HS.com requires all candidates to sign a confidentiality form. Our agency only places satisfactory candidates on another HS.com job. If we are not satisfied with the candidate’s performance, he/she is placed in our “Do-Not-Hire” files. Any job that HS.com places me on will be held in the strictest confidence. This includes personal family matters as well as the names, addresses and/or telephone numbers of family members. Only HS.com will be informed of any difficulties pertaining to the job.
I have told HouseholdStaffing.com that I am looking for a long-term position. I am also aware that HouseholdStaffing.com will not place anyone looking for temporary positions. If no emergencies arise, and the relationship between my employer and myself is satisfactory, I will remain on the job over a year and possibly for years.
It is further understood that the CANDIDATE is not employed by HS.com and that HS.com provides an introduction and referral service only and his/her status herein is that of an independent contractor. HS.com assumes no responsibility whatsoever for any actions concerning the placement, nor compliance with state or federal laws concerning employment, immigration, residency, insurance, taxes, or other legal requirements, all of which shall be the sole and exclusive responsibility of the CANDIDATE and for which CANDIDATE agrees to fully indemnify and hold harmless HS.com. Also, all payments for City, State and federal taxes, in connection with candidate’s employment herein, shall be the sole responsibility of candidate.
